At Future Health Management.LLC, we offer online telehealth services allowing our members to engage with medical professionals to obtain the medical and healthcare services they need. By accessing and using the Site, you are agreeing to be bound by the Terms and Conditions of Use and all other terms and policies that appear on this website. If you do not want to be bound by any of these Terms and Conditions of use, please cease use of the Site or its Services.

Healthcare Services

The medical professionals who deliver services through Future Health Management.LLC are independent professionals practicing medicine and healthcare. Future Health Management.LLC, by itself, does not practice or interfere with the practice of medicine or any other licensed profession by healthcare professionals, each of whom is responsible for his or her services and compliance with the requirements applicable to his or her profession and license. Neither Future Health Management.LLC nor any third party that promotes the services or provides you with a link to the services shall be liable for any professional advice you obtain from a Medical Professional via the services.

Site Content

None of the site content, other than the information you receive from medical Professionals, should be considered medical advice or an endorsement, representation or warranty that any particular medicine or treatment is safe, appropriate or effective for you.

User Accounts

When you register on the website, you must create an Account by entering your name, email address, username, password and other information collected by Future Health Management.LLC. To create an account, we assume that you are of legal age to form a binding contract. If you are not of legal age to form a binding contract, you may not register to use our services. You agree that the account information that you provide us at any and all times including during registration will be true, accurate and complete and current. You may not create more than one account or transfer or share your account password with anyone. Please be responsible for maintaining the confidentiality of your Account password. Fees And Purchase Terms

You agree to pay all fees or charges to your Account in accordance with the terms in effect at the time a fee or charge is due and payable. By providing Future Health Management.LLC with your credit card number or bank account number, PayPal or Venmo account and associated payment information, you agree that Future Health Management.LLC is authorized to immediately invoice your account for all fees and charges due and payable to Future Health Management.LLC hereunder and that no additional notice or consent is required. If your health plan, employer or agency has arranged with Future Health Management.LLC to pay the fee or any portion of the fee, or if the fee is pursuant to some other arrangement with Future Health Management.LLC, that fee adjustment will be reflected in the fee that you are ultimately charged. Please check with any employer or health plan or agency to determine if Services will be reimbursed.

You acknowledge that you are personally responsible or all incurred expenses if you do not have insurance coverage for fees or if the coverage is denied. Future Health Management.LLC offers no guarantee that you shall receive any such reimbursement. Future Health Management.LLC reserves the right to modify or implement a new pricing structure at any time prior to billing you for your initial payment or for future payments due pursuant to these Terms and Conditions of Use. You understand and agree that for Services provided on an appointment basis, you will be responsible for a missed appointment fee equal to all or a portion of the fees you and your insurer or other payor would have paid for the scheduled services if you do not cancel a scheduled appointment at least 24 hours in advance, unless we notify you in writing that a shorter cancellation window applies.

Indemnification And Limitation Of Liability

You agree to indemnify, defend and hold Future Health Management.LLC, its officers, directors, employees, healthcare Professionals, suppliers, and agents harmless from and against any claims, actions or demands, losses, liabilities, damages, costs, expenses, settlements (including without limitation reasonable attorney sand accounting fees), resulting from or alleged to result from directly or indirectly your violation of these Terms and Conditions, access to or use of the Platform and its Site, and provision of other disclosure to Future Health Management.LLC of any other information or data and the use of the same by Future Health Management.LLC or other Future Health Management.LLC Party as stated here under.

Future Health Management.LLC shall not be liable for any failure to perform its obligations hereunder where the failure results from any cause beyond Future Health Management.LLC’s reasonable control, including, without limitation, mechanical, electronic or communications failure or degradation.

We strongly recommend that you do not use the services via public computers. We also recommend that you do not store your account password through your web browser or software.

Continued use of the Site and Services following notice of such changes will indicate your acknowledgement of such changes and agreement to be bound by the revised Terms and Conditions of Use, inclusive of such changes.
